Case Management
Case management services are directed towards removing barriers to community living with people who have a disability these services include, but are not limited to:
- Individualized case plans unique to each person's needs, barriers, and strengths.
- Provide assistance with linkage and access to available resources and advocacy.
- Independent living skills, peer support, and transitions.
- Transition into greater independent living environments through individualized, consumer directed case management plans.
We here at CORD presume the confindence of all of our consumers and we rely on them to let us know if they need further assistance such as; Filling out forms/applications and/or accompanying consumers to sheduled appointments or meetings outside of the CORD office. Assistance with housing searches is also available if the persons are physically or intellectually unable to do this task themselves.
